چکیده

A 68-year-old woman with long-standing severe pulmonary arterial hypertension was referred for coronary catheterization for investigation of worsening exertional dyspnea associated with angina. Coronary angiography revealed critical ostial left main coronary artery (LMCA) stenosis. Given the history of severe pulmonary arterial hypertension, simultaneous angiography of the LMCA and main pulmonary artery (MPA) was performed, which suggested a severely dilated MPA compressing the ostial to mid-LMCA, causing a critical stenosis (Figure 1A). This correlated well with coronary

One-stage Hybrid Procedure for Patients with Valvular Pulmonary Stenosis and Coronary Artery Disease

A 68‐year‐old male was diagnosed congenital pulmonary stenosis (PS) and unstable angina. He had a history of chronic obstructive pulmonary disease (COPD). Echocardiography revealed dilated pulmonary artery branch and severe valvular PS. Transvalvular peak systolic pressure gradient (PG) was 64 mmHg (1 mmHg = 0.133 KPa). Coronary angiography demonstrated 75% narrowing of left main coronary (LM),...

A Case of Extrinsic Compression of the Left Main Coronary Artery Secondary to Pulmonary Artery Dilatation

Extrinsic compression of the left main coronary artery (LMCA) secondary to pulmonary artery dilatation is a rare syndrome. Most cases of pulmonary artery hypertension but no atherosclerotic risk factors rarely undergo coronary angiography, and hence, diagnoses are seldom made and proper management is often delayed in these patients. We describe a patient that presented with pulmonary hypertensi...

Different Presentation of Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery in Adults: Case Reports

Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital cardiac malformation. We report three cases of ALCAPA who survived to adulthood. The first case was a 51-year-old woman who complained of typical chest pain that was diagnosed with ALCAPA using cardiac catheterization and coronary computed tomographic angiography (CTA). The second case was a 30-...

Total Occlusion of Left Main Coronary Artery by Dilated Main Pulmonary Artery in a Patient with Severe Pulmonary Hypertension

A 34-year-old woman was admitted to the hospital because of recently aggravated right heart failure without angina for 5 months. When she was 25 years old, patch repair with Polytetrafluoroethylene (PTFE) was performed for the secondum type of atrial septal defect (ASD) with moderate pulmonary hypertension. The chest PA, echocardiography and cardiac catheterization at current admission revealed...

Relationship between the location of the most severe myocardial perfusion defects, the most severe coronary artery stenosis and the site of subsequent myocardial infarction [Persian]

This study evaluated the relationship between the location of the most severe coronary artery stenosis and the subsequent acute myocardial infarction (AMI). Methods: Of 1590 patients who were admitted with a diagnosis of AMI, we identified 44 patients who had undergone previous myocardial perfusion SPECT. Thirty-one of these patients also had previous coronary angiography. The relationshi...
